From the base of the penis that is facing towards you. Don't push the ruler or whatever into the skin, just lay it flat against the front and measure to the tip.
Girth- use paper/tape measure/string, wrap it round your dick, mark where the end of the string/paper touches the rest of the paper, unfold, measure.
